Long-Term Condition Reviews

Our practice nurses or doctors are experienced in dealing with the conditions listed below. Please contact reception if you feel you need a review.

  • Asthma
  • Diabetes
  • Vascular Clinic (angina/stroke/artery problems)
  • Hypothyroidism - When your Levothyroxine or Thyroxine tablets are nearing the end of the year's repeat prescription, please book a "Thyroid Blood Test" appointment without seeing a doctor. The results will go to your usual doctor who will then authorise another year's prescription, or ring you to alter the dosage.

People with diabetes, angina, stroke or artery problems will be called by letter once a year. Please make sure you have your blood test a week or so before the appointment.

Stop Smoking Clinic

Many people in this surgery have tried it and succeeded! Book in for the clinic at reception if you want help "packing up".
